This is for a tooth that has never had a root canal before, the nerve is either hurting you, or it's half dead, or it's completely dead.
Just removing the nerve or cleaning out the dead infected nerve will most definitely alleviate your discomfort. However, it does take time to heal afterwards, but hopefully it heals well so that you can keep your tooth for years ahead.
This is for a tooth that has had a root canal treatment before (you can see white fillings within the nerve canal, in this case, the previous provider separated 2 long files in the canals) and it's failing/reinfected.
Not all root canals can heal, those that are reinfected, we can try to redo it, and see if that helps! If it does, great!

You can go to many dentists, each will have a different opinion of how to treat your case. Regardless what the opinions may be, if they have the SAME diagnosis, then at least you will be well informed of what is your problem so you can choose the treatment plan that fits your desire.
However, IMPROPER diagnosis will lead to all sorts of IMPROPER treatment plan, which may (luckily) or may not (usually) not resolve your chief complaints!
Once diagnosis has been found, treatment options are given to you to choose what's best for you and your situation.
Options include:
1. Do Nothing (your option, we don't offer this option)
2. Conservative treatment (either endodontic treatment if possible)
3. Alternative treatment (if endo is possible but you don't want to, then you can extract the tooth and implant/bridge/leave the space)
Of course there are many other options for specific cases, but these are the most common. Sometimes patients come in with an obvious infection, then treatment of either endo or extraction is recommended.
It's not good to buy time with antibiotics, as if there's a problem, treat the cause, not drag on or buy time.
Nothing is guaranteed. We are all humans. We can only do things that mostly work, but give you possibilities of what can happen if things don't work!
Not all treatments heal 100%
Not all treatments will last months or years! (you must decide what you want)
Not all treatments are indicated. (Root fracture cases tend to be recommended for extractions, but there are alternative treatments like amputating the fractured root, hemisectioning the tooth and using the stronger side of the tooth, if it heals well, etc. -BUT ONLY IF YOU ARE UP FOR HERO-DONTICS
I normally tell patients, if you don't try, I won't know if the case will work or not. But if you want to try, I can't really tell you how the tooth/case will turn out. It could work, or it could not work. I can only try, the rest is up to your body to heal.
